When Does a Child Need a Tooth Pulled?

Most baby teeth fall out on their own, and permanent teeth are meant to last a lifetime. But sometimes a tooth needs a little help coming out, or it needs to be removed to protect your child’s overall oral health. Our pediatric dentists will always explore every option before recommending an extraction.

Common reasons a child may need a tooth extraction include severe tooth decay that a filling or crown cannot repair, infection or abscess that does not respond to other treatments, crowding from baby teeth that have not fallen out in time, dental trauma from injuries, and orthodontic preparation to create space for braces.

What Happens During an Extraction

Numbing & Comfort

We apply a topical numbing gel first, then use a local anesthetic so your child will not feel any pain. Sedation options are also available for children who need extra help relaxing.

Gentle Removal

The dentist gently rocks the tooth back and forth to widen the socket. Your child may feel some pressure, but no sharp pain. Once loose, the tooth is carefully lifted out—for most baby teeth, this takes just a few seconds.

Recovery & Aftercare

We place gauze over the area to help a blood clot form. Before you leave, we walk you through everything you need to know about caring for the extraction site at home so healing goes smoothly.

We Take Extra Care With Every Extraction

Many children tell us it was much easier than they expected. Our team uses kid-friendly language, a warm and patient approach, and plenty of encouragement to help your child feel brave and comfortable throughout the entire process.

Extractions, Aftercare & What to Know

Everything parents need to know about pediatric tooth extractions.

Severe Tooth Decay

When a cavity has damaged the tooth beyond what a filling or crown can repair, removal may be the best path forward.

Infection or Abscess

A tooth with a serious infection that does not respond to other treatments may need extraction to prevent spreading.

Crowding & Orthodontic Prep

Baby teeth that have not fallen out in time, or teeth that need removal to create space for braces or orthodontic treatment.

Soft Foods & Recovery Diet

Yogurt, applesauce, mashed potatoes, smoothies, and soup for the first day or two. Avoid straws, chips, nuts, and popcorn until healed.

Managing Discomfort

Over-the-counter children's pain medication and cold compresses applied 10 minutes on, 10 minutes off to reduce swelling.

Space Maintainers

If a baby tooth is lost early, a custom-fitted space maintainer may hold the gap open until the permanent tooth is ready to come in. Learn more about space maintainers.

Common Questions About Dental Sealants

Answers from our board-certified pediatric dentists.

Will my child feel pain during the extraction?
No. We thoroughly numb the area before the procedure begins, so your child will not feel any sharp pain. They may feel some pressure or wiggling, but it should not hurt. For children who are especially nervous, we also offer sedation options to help them stay calm and comfortable throughout the process.
Most children bounce back within one to two days. The extraction site itself typically heals within a week or so. Your child can usually return to school the next day, though we recommend avoiding strenuous physical activity for 24 to 48 hours. We will send you home with clear aftercare instructions to make recovery as smooth as possible.
Permanent teeth are only extracted when absolutely necessary, such as in cases of severe damage, infection, or orthodontic need. The procedure is similar but may take a bit longer. After a permanent tooth is removed, we will discuss options for maintaining proper spacing and alignment, which may include a space maintainer or orthodontic referral depending on your child’s age and dental development.
Dental anxiety is completely normal, and our team is experienced at helping nervous children feel at ease. We use gentle language, explain each step in a kid-friendly way, and give your child a sense of control throughout the visit. If your child needs extra support, we offer sedation dentistry options ranging from mild nitrous oxide to deeper sedation for more involved procedures.
If a baby tooth is lost earlier than expected, the surrounding teeth can shift into the empty space, which can crowd out the permanent tooth and lead to alignment problems. A space maintainer is a small, custom-fitted device that holds the gap open until the permanent tooth is ready. Not every extraction requires one, but our dentists will evaluate your child’s specific situation and let you know. Learn more about space maintainers.
